Overview

**BI 860585 + exemestane** is an investigational oral combination regimen consisting of BI 860585, a potent and selective dual mTORC1/2 (mechanistic target of rapamycin complex 1 and 2) inhibitor, administered together with exemestane, a steroidal aromatase inhibitor. BI 860585 inhibits both mTORC1 and mTORC2 kinase activity, aiming to overcome resistance limited by mTORC1 inhibition alone and disrupt pathways central to tumor proliferation, survival, and metabolism. Exemestane blocks the aromatase enzyme, reducing estrogen production and is standard therapy for hormone receptor-positive breast cancer. The combination is under investigation for advanced/metastatic solid tumors, especially hormone receptor-positive breast cancer, with the goal of improving outcomes over existing mTORC1 plus exemestane regimens.
