BI 905711 is a first-in-class, tetravalent bispecific monoclonal antibody developed by Boehringer Ingelheim for the treatment of gastrointestinal (GI) cancers. It is designed to simultaneously target and cross-link tumor necrosis factor-related apoptosis-inducing ligand receptor 2 (TRAILR2) and cadherin 17 (CDH17), a membrane protein highly expressed in GI tumors but not in normal hepatocytes. By cross-linking TRAILR2 with CDH17 on tumor cells, BI 905711 induces CDH17-dependent TRAILR2 oligomerization, which activates caspase-mediated apoptosis selectively in cancer cells while sparing healthy liver tissue. Preclinical studies have demonstrated potent and selective antitumor activity in CDH17-positive colorectal cancer models both *in vitro* and *in vivo*. The drug is currently being evaluated in phase I clinical trials for advanced GI cancers including gastric adenocarcinoma, metastatic colorectal adenocarcinoma, esophageal adenocarcinoma, and pancreatic ductal adenocarcinoma.
