BI1819479 is a potent and selective small molecule inhibitor of autotaxin (ATX), also known as ectonucleotide pyrophosphatase/phosphodiesterase 2 (ENPP2). Developed by Boehringer Ingelheim, the compound is designed to treat idiopathic pulmonary fibrosis (IPF) by targeting the ATX-LPA signaling pathway. Autotaxin is the primary enzyme responsible for generating lysophosphatidic acid (LPA) from lysophosphatidylcholine. LPA is a bioactive lipid that signals through specific G protein-coupled receptors to promote fibroblast recruitment, myofibroblast differentiation, and the production of extracellular matrix proteins. By inhibiting ATX, BI1819479 reduces circulating and local LPA levels, thereby potentially slowing the progression of lung tissue scarring and preserving pulmonary function in patients with IPF. Clinical development has included Phase 1 studies evaluating safety, tolerability, and pharmacokinetics in healthy volunteers and patients with IPF.
