BI3810477 is a high-affinity monoclonal antibody developed by Boehringer Ingelheim that targets connective tissue growth factor (CTGF/CCN2) for the treatment of idiopathic pulmonary fibrosis (IPF). It is designed to bind a specific conformational epitope on CTGF, which prevents its cleavage by matrix metalloproteinase 7 (MMP7) into pro-fibrotic N-terminal fragments. By disrupting this CTGF-MMP7 proteolytic axis, BI3810477 inhibits myofibroblast activation and mesenchymal migration while restoring alveolar cell fate. In preclinical patient-derived models, BI3810477 has demonstrated superior binding affinity and efficacy compared to the first-generation anti-CTGF antibody pamrevlumab.
