Overview

Biapenem is a parenteral carbapenem antibiotic with a broad spectrum of activity against Gram-positive and Gram-negative aerobic and anaerobic bacteria. It was developed by Meiji Seika and Lederle Laboratories. The drug functions by inhibiting bacterial cell wall synthesis through high-affinity binding to penicillin-binding proteins (PBPs), specifically PBP 1, 2, 3, and 4, which leads to the disruption of peptidoglycan cross-linking and subsequent bacterial cell lysis. A distinguishing feature of biapenem compared to earlier carbapenems like imipenem is its high stability against hydrolysis by human renal dehydropeptidase-I (DHP-I), which eliminates the need for co-administration with a DHP-I inhibitor. It is primarily used for treating severe infections such as pneumonia, complicated urinary tract infections, and intra-abdominal infections.
