Overview

BIBW 2948 BS is an inhaled small molecule antagonist of the epidermal growth factor receptor (EGFR), developed for the treatment of chronic obstructive pulmonary disease (COPD) associated with chronic bronchitis. The drug was designed to inhibit EGFR activation in airway epithelial cells, a pathway implicated in mucin hypersecretion and airway remodeling in COPD. Clinical studies demonstrated that inhaled BIBW 2948 BS produced dose-dependent inhibition of EGFR internalization but did not significantly reduce epithelial mucin stores compared to placebo over four weeks. Higher doses showed greater EGFR inhibition but were associated with increased adverse events, including reversible liver enzyme elevations and reductions in lung function (FEV1). Development was discontinued due to poor tolerability and lack of significant clinical efficacy[1][2][4][6].
