Bicarbonate dialysate is a sterile solution used as the dialysis fluid in hemodialysis and continuous renal replacement therapy (CRRT). It contains sodium bicarbonate as its primary buffer to correct metabolic acidosis in patients with kidney failure. The mechanism of action involves diffusion of bicarbonate from the dialysate into the patient's blood across a semipermeable membrane during dialysis, thereby restoring acid-base balance. Bicarbonate dialysates are now preferred over acetate-based solutions due to better biocompatibility and improved patient outcomes. The composition can be tailored according to clinical needs and may include other electrolytes such as potassium, calcium, magnesium, chloride, and sometimes phosphate[1][2][3][6][9]. Commercially available products include ready-to-use two-chamber bags or concentrates for mixing on-site.
