Overview
**Bicisate** is a lipophilic small-molecule ligand used in the diagnostic radiopharmaceutical product **Neurolite**. After reconstitution and chelation with technetium Tc 99m, technetium Tc 99m bicisate passively diffuses across intact cell membranes and the blood-brain barrier; stereospecific intracellular de-esterification then promotes cerebral retention in proportion to regional cerebral blood flow. It is administered intravenously for SPECT imaging as an adjunct to CT or MRI for localization of stroke in patients with an established stroke diagnosis.
