Biktarvy is a fixed-dose combination tablet containing bictegravir, emtricitabine, and tenofovir alafenamide. Bictegravir is a second-generation integrase strand transfer inhibitor (INSTI) with a high genetic barrier to resistance and a favorable safety profile. Emtricitabine and tenofovir alafenamide are nucleoside reverse transcriptase inhibitors (NRTIs) that inhibit viral replication by acting as chain terminators. Developed by Gilead Sciences, Biktarvy is indicated as a complete single-tablet regimen for the treatment of HIV-1 infection in adults and pediatric patients. It is notable for its high efficacy, lack of requirement for a pharmacokinetic booster, and minimal drug-drug interactions compared to earlier protease inhibitor or cobicistat-boosted regimens. Clinical investigations continue to evaluate its use in infants, as post-exposure prophylaxis (PEP), and in patients with pre-existing NRTI resistance.
