Biejiajianwan is a classic Traditional Chinese Medicine (TCM) formula, first recorded in the Han Dynasty text *Synopsis of Prescriptions of the Golden Chamber* (Jin Gui Yao Lue), primarily used for the treatment of abdominal masses and liver-related disorders. It is a complex multi-ingredient preparation traditionally composed of 23 components, with Carapax Trionycis (turtle shell) as the sovereign medicinal, alongside other ingredients such as Donkey-hide gelatin, Rheum palmatum, and Scutellaria baicalensis. The formula is indicated for "softening hardness and dissipating masses" (Ruan Jian San Jie), specifically targeting hepatic fibrosis, liver cirrhosis, and splenomegaly. Modern pharmacological studies suggest it works by inhibiting the TGF-beta1/Smad signaling pathway, reducing the activation of hepatic stellate cells, and promoting the degradation of the extracellular matrix. It is widely manufactured in China, notably by Sinopharm Group Zhonglian Pharmaceutical, and is frequently used as an adjunctive therapy for chronic hepatitis B-related fibrosis and cirrhosis.
