Biglucem AP

Biglucem AP is an extended-release oral formulation of metformin hydrochloride developed and marketed by Laboratorio SIDUS in Argentina. It is indicated for the treatment of type 2 diabetes mellitus, particularly in overweight patients, when dietary management and physical exercise alone do not achieve adequate glycemic control. As a biguanide antihyperglycemic agent, metformin works through multiple pathways to lower blood glucose levels without stimulating insulin secretion or causing hypoglycemia. Its primary mechanism of action involves the inhibition of mitochondrial complex I in hepatocytes, which decreases cellular ATP production and increases the AMP:ATP ratio. This energy stress activates AMP-activated protein kinase (AMPK), leading to the downregulation of gluconeogenic genes and a subsequent reduction in hepatic glucose output. Additionally, metformin enhances peripheral insulin sensitivity, improving glucose uptake and utilization in skeletal muscle, and decreases intestinal glucose absorption. The extended-release formulation of Biglucem AP allows for once-daily dosing, which improves patient adherence and reduces gastrointestinal side effects compared to immediate-release formulations.
