Bimatoprost implant is a sustained-release formulation of bimatoprost, a prostaglandin analog used to reduce intraocular pressure (IOP) in patients with open-angle glaucoma or ocular hypertension. The first-in-class version, Durysta, is a 10 mcg biodegradable intracameral implant developed by Allergan (AbbVie) that provides continuous drug delivery for several months. Newer iterations, such as the system developed by SpyGlass Pharma, utilize a drug-eluting platform (78 mcg) integrated with an intraocular lens (IOL) during cataract surgery to provide long-term IOP control. Bimatoprost acts as a potent agonist at the prostaglandin F2α (FP) receptor, which increases the outflow of aqueous humor through both the trabecular meshwork and uveoscleral pathways. These implants are designed to overcome the significant challenges of patient non-compliance and the ocular surface toxicity associated with daily topical eye drops.
