Overview

Bimixin is a combination oral antibiotic therapy consisting of neomycin and bacitracin, specifically utilized as a non-absorbable antimicrobial prophylaxis. Neomycin is an aminoglycoside that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, while bacitracin is a polypeptide antibiotic that disrupts bacterial cell wall synthesis by interfering with the dephosphorylation of C55-isoprenyl pyrophosphate. Because these components are not absorbed by the gastrointestinal mucosa, Bimixin acts locally within the gut lumen. In the COLORAL1 clinical trial led by the University of Turin, Bimixin is being evaluated for its efficacy in reducing colonic bacterial load and preventing surgical site infections (SSI) in patients undergoing elective colorectal surgery, typically administered preoperatively in conjunction with standard intravenous antibiotic prophylaxis.
