BIOD-123 is an ultra-rapid-acting formulation of recombinant human insulin (RHI) developed for use as a mealtime (prandial) insulin in patients with diabetes mellitus, primarily type 1 diabetes. It was originally developed by Biodel and later by Albireo Pharma, which became a subsidiary of Ipsen. The drug is designed to be absorbed more quickly than currently marketed rapid-acting insulins such as insulin lispro (Humalog), aiming to better mimic physiological insulin release after meals. Clinical trials have shown that BIOD-123 achieves glycemic control comparable to Humalog, with some trends toward reduced postprandial glucose excursions and potentially less weight gain during stable dosing periods. The most notable adverse event was increased injection site pain, which was generally mild and transient[1][2][5][6].
