**Biofermin R** is a Japanese prescription intestinal-regulating probiotic preparation developed and manufactured by Biofermin Seiyaku. Its active constituent is the multi-antibiotic-resistant lactic acid bacterial strain *Enterococcus faecium* 129 BIO 3B-R. The live organism can proliferate during treatment with susceptible antibacterial agents, producing lactic acid and helping restore antibiotic-disrupted intestinal microbiota while suppressing overgrowth of potentially harmful organisms. It is marketed in oral powder and tablet formulations for improvement of gastrointestinal symptoms resulting from intestinal-flora abnormalities during administration of specified antibacterial or chemotherapeutic agents. It has also been studied as adjunctive treatment during *Helicobacter pylori* eradication therapy to reduce antibiotic-associated gastrointestinal adverse events.
