Overview

Biphasic insulin premix refers to a class of insulin formulations that combine a rapid-acting or short-acting insulin with an intermediate-acting (protaminated) insulin component in a single injection. This dual-action profile provides both prandial (mealtime) coverage to control postprandial glucose spikes and basal (background) coverage to manage fasting glucose levels. These formulations are primarily used in the management of type 1 and type 2 diabetes mellitus. By binding to the insulin receptor, they facilitate glucose uptake into cells and inhibit hepatic glucose production. Common examples include biphasic insulin aspart (e.g., NovoMix 30) and biphasic human insulin (e.g., Mixtard 30/70). They are typically administered subcutaneously, often once or twice daily, to simplify insulin regimens for patients requiring both basal and bolus support.
