Birinapant + docetaxel is a combination therapy consisting of birinapant, a small molecule SMAC (Second Mitochondria-derived Activator of Caspase) mimetic and antagonist of inhibitor of apoptosis proteins (IAPs), and docetaxel, a taxane-class chemotherapeutic agent. Birinapant sensitizes tumor cells to apoptosis by promoting degradation of cIAP1/2 and XIAP, thereby facilitating caspase activation and cell death, especially in the presence of tumor necrosis factor (TNF). Docetaxel induces apoptosis primarily by stabilizing microtubules and interfering with cell division, and it can also induce TNF production, further enhancing the pro-apoptotic effect of birinapant. This combination has shown synergistic antitumor activity in preclinical models of triple-negative breast cancer (TNBC) and head and neck squamous cell carcinoma (HNSCC), including xenografts resistant to docetaxel monotherapy. Clinical trials have evaluated this combination for safety and efficacy in patients with advanced or metastatic solid tumors[1][2][4].
