**Birinapant + irinotecan** is an investigational pharmaceutical drug combination used in oncology research. Birinapant is a small molecule SMAC mimetic that antagonizes inhibitor of apoptosis proteins (IAPs) including cIAP1 and XIAP, promoting apoptosis in cancer cells. Irinotecan is a topoisomerase I inhibitor that prevents DNA unwinding, causing DNA damage and cell death in rapidly dividing cells. Combination therapy is being studied for synergistic antitumor effects, where birinapant sensitizes cancer cells—particularly colorectal and other solid tumors—to chemotherapy-induced cell death by restoring apoptotic signaling and overcoming chemoresistance, potentially enhancing the cytotoxicity of irinotecan.
