**Birinapant + liposomal doxorubicin** is a combination regimen of two investigational drugs for oncology. **Birinapant** is a synthetic Smac-mimetic that inhibits inhibitor of apoptosis proteins (IAPs), thereby enabling activation of caspases and promoting apoptosis, especially in tumor cells. **Liposomal doxorubicin** is a pegylated liposomal formulation of the anthracycline doxorubicin, designed for improved pharmacokinetics and reduced cardiotoxicity, acting primarily as a DNA intercalator and inhibitor of topoisomerase II, leading to DNA damage and cell death. The combination is being studied for solid and hematological tumors, including ovarian cancers, due to demonstrated preclinical synergy between IAP inhibition and DNA-damaging chemotherapy. Phase I/II clinical studies have evaluated its tolerability, dose escalation, and preliminary efficacy in relapsed/refractory solid tumors, with promising indications for platinum- and anthracycline-resistant populations[1][4][7][8].
