Bisacodyl is a stimulant laxative used for the short-term treatment of constipation and to empty the bowels before surgery or certain medical procedures. It acts directly on the colon by stimulating sensory nerve endings in the intestinal mucosa to increase peristalsis and promote bowel movements. Bisacodyl is also used in managing neurogenic bowel dysfunction and as part of bowel preparation regimens prior to diagnostic procedures such as colonoscopy. Its mechanism involves direct activation of colonic macrophages leading to increased prostaglandin E2 (PGE2) secretion and reduced expression of aquaporin-3 (AQP3), resulting in enhanced water secretion into the colon[1][2][6]. Bisacodyl is a derivative of triphenylmethane and has been widely available since 1953[6].
