Overview

Bismuth subcitrate is a bismuth salt used primarily in combination with antibiotics and a proton pump inhibitor for the treatment of *Helicobacter pylori* infections, which are associated with peptic ulcers and gastritis. It is often formulated as part of a fixed-dose combination (notably under the brand name Pylera) with metronidazole and tetracycline. Bismuth subcitrate acts through several mechanisms including inhibition of peptic activity, increasing mucus glycoprotein secretion, binding to the gastric mucus layer to act as a barrier against hydrochloric acid, accelerating ulcer healing by promoting accumulation of epidermal growth factor at ulcer sites, exerting cytoprotective effects by increasing mucosal prostaglandins and bicarbonate secretion, and exhibiting bactericidal activity against *H. pylori*. It also prevents adhesion of *H. pylori* to epithelial cells and inhibits bacterial enzymes such as proteases, lipases, glycosidases, and phospholipases[1][5].
