The bispecific antibody-drug conjugate (bsADC) targeting CEACAM5 and CDH17 is an experimental therapeutic agent developed by Biotheus for the treatment of gastrointestinal (GI) malignancies. It consists of a bispecific antibody that simultaneously binds to Carcinoembryonic Antigen-Related Cell Adhesion Molecule 5 (CEACAM5) and Cadherin-17 (CDH17), conjugated to a cytotoxic topoisomerase I inhibitor payload. These two antigens are highly and frequently co-expressed in over 95% of colorectal cancers, as well as in a high percentage of gastric, pancreatic, and cholangiocarcinomas, while having limited expression in normal tissues. By targeting both antigens, the bsADC aims to achieve enhanced tumor selectivity and potent anti-tumor activity, including bystander killing effects, even in tumors with varying expression levels of the individual targets. Preclinical studies have demonstrated superior or comparable efficacy to benchmark monospecific ADCs targeting either CEACAM5 or CDH17, along with a favorable pharmacokinetic profile and tolerability in animal models.
