Bispecific BCMAxCD3 antibodies are a class of immunotherapy agents designed to treat multiple myeloma by redirecting the immune system to attack cancer cells. These molecules possess dual binding specificity: one arm binds to the B-cell maturation antigen (BCMA), which is highly and selectively expressed on the surface of malignant plasma cells, while the other arm binds to the CD3 receptor on T cells. By physically bridging these two cell types, the antibody triggers T-cell activation and the release of cytotoxic proteins like perforin and granzymes, leading to the direct lysis of the myeloma cells. This mechanism of action is independent of major histocompatibility complex (MHC) recognition, allowing for potent anti-tumor activity even in tumors that have downregulated MHC expression. Several agents in this class, such as teclistamab and elranatamab, have received regulatory approval for the treatment of relapsed or refractory multiple myeloma.
