This drug is a **combination immunosuppressive regimen** comprising four agents: - **BL-8040:** A novel CXCR4 antagonist/peptide drug under investigation for hematologic malignancies and immune modulation. - **Horse anti-thymocyte globulin (brand name Atgam):** A polyclonal immune globulin (primarily IgG) derived from horse serum immunized with human thymocytes; used to induce immunosuppression mainly via T cell depletion, complement-mediated lysis, and T lymphocyte apoptosis, along with potential stimulation of hematopoietic growth factors[1][3][5]. - **Methylprednisolone:** A synthetic glucocorticoid corticosteroid, acts both as an anti-inflammatory and immunosuppressant by modulating gene transcription via the glucocorticoid receptor. - **Cyclosporine:** A calcineurin inhibitor that selectively inhibits T cell activation by blocking the dephosphorylation and activation of NF-AT, thereby reducing interleukin-2 and other cytokine production[2][4][6]. The combination is typically used to profoundly suppress immune responses, such as in transplantation (especially hematopoietic stem cell and solid organ), autoimmune diseases, or specialized trial regimens for refractory or severe conditions.
