Overview

Bleomycin + OK-432 is a combination sclerotherapy regimen investigated for the treatment of parotid salivary fistulas, particularly those refractory to conservative management. Bleomycin is a cytotoxic glycopeptide antibiotic that induces DNA strand breaks and subsequent fibrosis of secretory tissue. OK-432 (Picibanil) is a lyophilized preparation of Streptococcus pyogenes (Group A, type 3) treated with penicillin G, which acts as a biological response modifier. It triggers a localized inflammatory response and cytokine release (such as IL-1, IL-2, and TNF-alpha) via TLR4 activation, promoting the obliteration of the fistula tract. This combination therapy, explored by researchers at Sun Yat-Sen Memorial Hospital, aims to provide a minimally invasive alternative to surgical intervention for salivary gland disorders.
