Overview

Blinatumomab is a bispecific T-cell engager (BiTE) antibody construct that targets CD19 on B cells and CD3 on T cells, thereby redirecting cytotoxic T cells to lyse malignant B cells. It is approved for the treatment of relapsed or refractory CD19-positive B-cell precursor acute lymphoblastic leukemia (ALL) in both adults and children. Donor lymphocyte infusion (DLI) involves the administration of lymphocytes from a stem cell transplant donor to enhance graft-versus-leukemia effects after allogeneic hematopoietic stem cell transplantation. The combination of blinatumomab with DLI aims to augment anti-leukemic activity by leveraging both direct T-cell mediated cytotoxicity and enhanced graft-versus-leukemia effects, particularly in patients who relapse after allogeneic transplantation. This approach has shown promise in case reports and early studies for controlling disease progression and achieving remission in relapsed ALL or mixed phenotype acute leukemia following transplant[1][3][4][5].
