Blinatumomab is a bispecific T-cell engager (BiTE) antibody construct that targets CD19 on B cells and CD3 on T cells, thereby redirecting cytotoxic T cells to lyse malignant B cells. It is approved for the treatment of relapsed or refractory CD19-positive B-cell precursor acute lymphoblastic leukemia (ALL) in both adults and children. Donor lymphocyte infusion (DLI) involves the administration of lymphocytes from a stem cell transplant donor to enhance graft-versus-leukemia effects after allogeneic hematopoietic stem cell transplantation. The combination of blinatumomab with DLI aims to augment anti-leukemic activity by leveraging both direct T-cell mediated cytotoxicity and enhanced graft-versus-leukemia effects, particularly in patients who relapse after allogeneic transplantation.
