Blinatumomab Expanded T-cells (BET) is an autologous cell therapy product developed by the Papa Giovanni XXIII Hospital for the treatment of CD20+ indolent non-Hodgkin lymphomas (iNHL) and chronic lymphocytic leukemia (CLL). The therapy involves the ex vivo expansion of a patient's own T cells (polyclonal CD4+ and CD8+ cells) using the bispecific T-cell engager (BiTE) blinatumomab and recombinant human interleukin-2 (rhIL-2). During the expansion process, blinatumomab cross-links CD3 on T cells with CD19 on contaminating neoplastic B cells, leading to the activation of T cells and the simultaneous elimination of tumor cells. The resulting Advanced Therapeutic Medicinal Product (ATMP) is infused back into the patient to facilitate immune reconstitution and potentially provide anti-tumor activity following immunosuppressive chemotherapy regimens like FCR (fludarabine, cyclophosphamide, rituximab) or BR (bendamustine, rituximab).
