A blood clot scaffold is an autologous biological matrix used in regenerative endodontic procedures to treat immature permanent teeth with necrotic pulp. The procedure involves the induction of periapical bleeding into the root canal space, typically by over-instrumentation with a dental file beyond the apex. The resulting blood clot serves as a natural three-dimensional fibrin scaffold that traps endogenous growth factors, such as platelet-derived growth factor (PDGF) and vascular endothelial growth factor (VEGF), and provides a microenvironment for the recruitment and differentiation of mesenchymal stem cells from the apical papilla. This regenerative approach aims to facilitate continued root development (apexogenesis) and thickening of the canal walls. Nourhan M. Aly is investigating the efficacy of this scaffold, both alone and in combination with Platelet-Rich Fibrin (PRF), in a Phase 2 clinical trial.
