Overview

The PRIEST trial (Prehospital Plasma or Red Blood Cell Transfusion Strategy in Major Bleeding), sponsored by the University of Aarhus, evaluates different prehospital transfusion strategies for patients with major hemorrhage. The intervention involves the administration of blood products, specifically comparing three strategies: a mixture of plasma and red blood cell (RBC) transfusion (standard of care), RBC transfusion alone, and plasma transfusion alone. The goal is to determine the superior strategy for treating circulatory shock (measured by base deficit) and preserving endothelial function and clot formation in emergency settings. Plasma provides essential coagulation factors and proteins to maintain osmotic pressure and hemostasis, while red blood cells restore the oxygen-carrying capacity of the blood.
