**BLUE-181** is a synthetic small molecule developed by Blue Therapeutics using their R2i platform, which targets receptor interactions (receptor-interactomics) to create novel drug targets called interaction modulators (iMods). It selectively modulates the **HM2K receptor** (a novel receptor interaction target) in the spinal cord to provide potent analgesia, reported to be 50 times more powerful than morphine, while avoiding brain receptors associated with addiction, euphoria, tolerance, and abuse potential. Primarily indicated for **pain** as a non-addictive opioid alternative, it also shows promise in **irritable bowel syndrome (IBS)** and anxiety disorders; preclinical pipeline exploration includes infectious diseases (e.g., SARS-CoV-2), breast cancer, and Parkinson's disease. As of 2021, it was approaching IND filing with strong preclinical efficacy data and no tolerance or addiction risk observed.[1][5][7]
