Overview

BM-104 is a near-infrared (NIR) fluorescent imaging agent developed by the Leiden University Medical Center (Academisch Ziekenhuis Leiden) for the intraoperative detection and visualization of colorectal malignancies. It consists of a high-affinity nanobody (a single-domain antibody fragment) that specifically targets Carcinoembryonic Antigen (CEA/CEACAM5), conjugated to the fluorescent dye IRDye 800CW. CEA is a well-established tumor-associated antigen that is overexpressed in over 90% of rectal adenocarcinomas and dysplastic colorectal polyps, while showing minimal expression in normal adult tissues. By binding to CEA-expressing cells, BM-104 allows surgeons to identify tumor margins, detect occult lymph node metastases, and distinguish between neoplastic and healthy tissue in real-time using NIR fluorescence imaging systems. This tracer is primarily being investigated for use in fluorescence-guided surgery (FGS) and advanced endoscopic procedures for early rectal cancer and colorectal adenomatous polyps.
