BMS-394136 is a small molecule investigational drug developed by Bristol Myers Squibb as a selective inhibitor of the atrial-specific potassium current IKur, which is encoded by the KCNA5 gene and mediated by the voltage-gated potassium channel Kv1.5. The compound was designed for the treatment of cardiac arrhythmias, particularly atrial fibrillation. Mechanistically, BMS-394136 blocks Kv1.5 channels in atrial tissue, prolonging the atrial effective refractory period (AERP) and action potential duration (APD) without affecting ventricular repolarization—demonstrating strong atrial selectivity. Preclinical studies and phase 1 clinical trials showed that its efficacy may be enhanced under conditions such as diabetes mellitus or acute atrial ischemia[2][4][6][8]. Despite promising early results regarding its safety and selectivity profile, further development was discontinued after phase 1 trials.
