DS003 (BMS-599793) is a small molecule entry inhibitor developed as an analogue of BMS-806 for the prevention of HIV-1 infection. It acts by binding to or near the CD4 binding site on the HIV-1 envelope glycoprotein gp120, thereby preventing the CD4-induced conformational changes essential for viral fusion and entry into host cells. This mechanism blocks HIV from infecting target cells. DS003 has demonstrated potent inhibitory activity against various HIV-1 clade B isolates and was selected for development as a topical microbicide, particularly as a vaginal tablet for pre-exposure prophylaxis (PrEP). The drug has advanced to Phase I clinical trials, showing good safety profiles and effective concentrations in genital tract tissues capable of inhibiting ex vivo HIV infection[1][5][7]. Its low systemic bioavailability supports its use as a localized microbicide rather than systemic therapy[5].
