BMS-986351 is an investigational bispecific antibody designed to target both CD40 and Fibroblast Activation Protein (FAP). It functions as a conditional CD40 agonist, meaning it is engineered to activate the CD40 receptor on antigen-presenting cells (APCs) primarily when it is cross-linked by binding to FAP, which is highly expressed on cancer-associated fibroblasts within the tumor microenvironment. This localized activation approach is intended to stimulate a robust anti-tumor immune response while minimizing the systemic toxicities—such as cytokine release syndrome and hepatotoxicity—that have historically limited the clinical utility of non-targeted CD40 agonists. Developed by Bristol Myers Squibb, BMS-986351 is currently in Phase 1 clinical trials for the treatment of advanced or metastatic solid tumors. Other companies, including Genentech (RG6189) and Sanofi (SAR445877), are also developing similar FAP-targeted CD40 bispecific antibodies.
