BMS-986489 is an orally bioavailable small molecule inhibitor of the Programmed Death-Ligand 1 (PD-L1), developed by Bristol Myers Squibb. Unlike monoclonal antibodies that target the PD-1/PD-L1 pathway via intravenous infusion, BMS-986489 is designed as an oral alternative that blocks the interaction between PD-L1 and its receptor, PD-1. By inhibiting this checkpoint, the drug relieves the suppression of T-cell activity, thereby enhancing the immune system's ability to recognize and eliminate tumor cells. As of 2024, it is being evaluated in Phase III clinical trials as a first-line treatment for extensive-stage small cell lung cancer (ES-SCLC) in combination with chemotherapy, where it is being compared against the standard-of-care PD-L1 inhibitor atezolizumab.
