Boceprevir + pegylated interferon alfa + ribavirin is a triple combination antiviral therapy used for the treatment of chronic hepatitis C virus (HCV) genotype 1 infection. Boceprevir is a first-generation NS3/4A serine protease inhibitor that directly inhibits HCV replication. Pegylated interferon alfa (peginterferon alfa-2a or -2b) is an immunomodulatory cytokine conjugated to polyethylene glycol to extend its half-life, enhancing antiviral immune responses. Ribavirin is a nucleoside analog with broad-spectrum antiviral activity, thought to act by inhibiting viral RNA synthesis and inducing lethal mutagenesis in viruses. This combination was indicated for adults with chronic HCV genotype 1 and compensated liver disease, including those previously untreated or who had failed prior therapy. The regimen involves an initial lead-in phase of pegylated interferon alfa and ribavirin followed by the addition of boceprevir[1][2][8]. Triple therapy significantly increased sustained virologic response rates compared to dual therapy but was associated with higher rates of adverse events such as anemia[5][8]. With the advent of newer direct-acting antivirals, this regimen has been largely supplanted in clinical practice.
