Bohm solution is a sterile, non-pyrogenic cardioplegic solution used primarily in cardiac surgery to induce and maintain elective cardiac arrest. Developed by Laboratorios Bohm, it is administered via the coronary arteries or the aortic root during cardiopulmonary bypass to protect the myocardium during the ischemic period of surgery. The solution's mechanism of action is multifaceted: it contains a high concentration of potassium ions to depolarize the myocardial cell membrane, thereby arresting the heart in diastole and significantly reducing metabolic demand. Additionally, it incorporates magnesium chloride, which acts as a physiological calcium antagonist to stabilize the membrane and prevent calcium overload, and procaine hydrochloride, a local anesthetic that blocks voltage-gated sodium channels to further inhibit electrical activity and provide additional myocardial protection.
