Bone marrow aspirate (BMA) is an autologous biological therapy being evaluated by the Instituto Nacional de Traumatologia e Ortopedia (INTO) for the treatment of knee osteoarthritis. BMA is obtained through aspiration from the patient's bone marrow (typically the iliac crest) and contains a concentrated population of mesenchymal stem cells (MSCs), hematopoietic stem cells, and various growth factors and anti-inflammatory cytokines, such as interleukin-1 receptor antagonist (IL-1Ra). When injected intra-articularly, these components are thought to modulate the joint microenvironment, reduce chronic inflammation, and potentially promote tissue homeostasis or stabilize cartilage degeneration. In a Phase 3 clinical trial conducted by INTO, BMA is being compared against a "BMA matrix" (a combination of BMA and hyaluronic acid) and hyaluronic acid alone to assess its efficacy in improving functional outcomes and reducing symptoms in patients with knee osteoarthritis.
