Bone marrow aspirate concentrate (BMAC) is an autologous cell therapy produced by concentrating a patient's own bone marrow aspirate using a point-of-care centrifugation system, such as the Harvest BMAC System developed by Harvest Technologies (now part of Terumo BCT). The resulting concentrate contains a heterogeneous population of nucleated cells, including mesenchymal stem cells (MSCs), hematopoietic stem cells (HSCs), and endothelial progenitor cells (EPCs), along with a concentrated mix of cytokines and growth factors. In the treatment of ischemic congestive heart failure, BMAC is typically administered via intramyocardial injection during coronary artery bypass grafting (CABG) surgery. The therapeutic effect is primarily mediated through paracrine mechanisms; the injected cells secrete various factors that promote angiogenesis, modulate the inflammatory response, and reduce apoptosis of cardiomyocytes, thereby improving myocardial perfusion and ventricular function.
