Overview

Bone marrow aspirate concentrate (BMAC) is an autologous biologic therapy being investigated by Shane A. Shapiro, primarily at the Mayo Clinic, for the treatment of shoulder osteoarthritis. The therapy is prepared by harvesting bone marrow from the patient's own iliac crest and concentrating it through centrifugation. This process enriches the concentration of mononuclear cells, including mesenchymal stem cells (MSCs), and a variety of bioactive growth factors such as platelet-derived growth factor (PDGF), transforming growth factor-beta (TGF-β), vascular endothelial growth factor (VEGF), and bone morphogenetic proteins (BMPs). In the specific clinical protocol (NCT04826224), BMAC is administered into the shoulder joint following a Comprehensive Arthroscopic Management (CAM) surgical procedure. The goal of the therapy is to modulate the inflammatory environment of the osteoarthritic joint and promote tissue repair through the paracrine effects of the concentrated cells and growth factors.
