Bone marrow-derived mesenchymal stem cells (BM-MSCs) are multipotent stromal cells harvested from the bone marrow that possess the ability to differentiate into various mesodermal lineages, including osteoblasts, chondrocytes, and adipocytes. These cells are characterized by their potent immunomodulatory, anti-inflammatory, and regenerative properties. BM-MSCs exert their therapeutic effects primarily through paracrine signaling, secreting a wide array of cytokines and neurotrophic factors (such as BDNF, NGF, and VEGF), and through direct cell-to-cell contact to modulate the activity of T cells, B cells, and macrophages. They are being extensively investigated in clinical trials for a diverse range of indications, including neurological disorders like amyotrophic lateral sclerosis (ALS) and ischemic stroke, inflammatory conditions such as Crohn's disease (perianal fistulae) and graft-versus-host disease (GVHD), and musculoskeletal injuries like tendinopathy and knee osteoarthritis.
