Bone marrow-derived mononuclear cells + granulocyte-colony stimulating factor (G-CSF) is an investigational autologous cell therapy combination being developed by Barts Health NHS Trust for the treatment of dilated cardiomyopathy (DCM). The therapy involves a two-step process: first, the administration of G-CSF (specifically filgrastim) to mobilize CD34+ progenitor cells from the bone marrow into the circulation; and second, the harvesting of autologous bone marrow-derived mononuclear cells (BMMNCs) via aspiration from the iliac crest. These cells are then processed and delivered to the patient via intracoronary infusion, often supported by percutaneous circulatory assistance (such as the Impella CP device) to maintain hemodynamic stability during the procedure. The biological rationale is to promote myocardial regeneration and repair through the paracrine effects and potential differentiation of the infused progenitor cells. This combination is currently being evaluated in the Phase II DCM-Support clinical trial.
