Bone marrow stem cells (BMSCs), specifically bone marrow-derived mesenchymal stem cells, are an investigational cell therapy being developed by the Second Affiliated Hospital, School of Medicine, Zhejiang University for the treatment of acute respiratory distress syndrome (ARDS). These multipotent cells are administered intravenously and function through complex immunomodulatory mechanisms. Upon administration, BMSCs migrate to the site of lung injury where they secrete a variety of paracrine factors, including anti-inflammatory cytokines and growth factors. These secretions help to suppress the cytokine storm associated with ARDS, reduce pulmonary edema by promoting alveolar fluid clearance, and facilitate the repair of damaged alveolar epithelial and endothelial cells. This therapy is currently being evaluated as part of the PETARDS platform trial, which utilizes an adaptive design to assess multiple interventions for ARDS patients in the intensive care unit.
