A specialized cell therapy regimen developed by Massachusetts General Hospital (MGH) to induce immune tolerance in patients who have previously received a kidney transplant. The protocol involves the transplantation of donor-derived bone marrow or peripheral blood stem cells to create a state of mixed chimerism, where the recipient's immune system coexists with donor-derived cells. This process is facilitated by a non-myeloablative conditioning regimen consisting of siplizumab (an anti-CD2 monoclonal antibody), rituximab (an anti-CD20 monoclonal antibody), cyclophosphamide, and fludarabine. The ultimate goal is to re-educate the recipient's immune system to recognize the donor organ as self, potentially allowing for the permanent discontinuation of immunosuppressive medications and reducing the risk of long-term rejection.
