Overview

Bonmax is a generic oral formulation of **alendronate sodium**, a nitrogen-containing bisphosphonate used primarily for the treatment and prevention of osteoporosis in postmenopausal women and to increase bone mass in men with osteoporosis. The drug functions by binding to bone hydroxyapatite crystals and being internalized by bone-resorbing osteoclasts. Once inside the osteoclast, alendronate inhibits the enzyme **farnesyl pyrophosphate synthase (FPPS)**, a key component of the mevalonate pathway. This inhibition prevents the prenylation of small GTP-binding proteins such as Rho, Rac, and Rab, which are critical for osteoclast function, cytoskeleton maintenance, and survival. Consequently, osteoclast activity is suppressed and apoptosis is induced, leading to a decrease in bone resorption and a significant increase in bone mineral density (BMD). Clinical evidence, including head-to-head trials against the reference brand Fosamax, has demonstrated that Bonmax provides equivalent gains in BMD and similar safety profiles.
