Overview

Velcade-BEAM is an experimental conditioning regimen developed by the University of Nebraska Medical Center for patients undergoing autologous hematopoietic stem cell transplantation (ASCT). It combines the proteasome inhibitor bortezomib (Velcade) with the standard BEAM chemotherapy backbone, which consists of carmustine (BCNU), etoposide, cytarabine (Ara-C), and melphalan. Bortezomib targets the 26S proteasome, inhibiting the NF-κB pathway to sensitize lymphoma cells to the cytotoxic effects of the alkylating agents and antimetabolites in the BEAM regimen. This combination is primarily investigated for the treatment of relapsed or refractory indolent non-Hodgkin lymphoma, transformed lymphoma, and mantle cell lymphoma, aiming to improve complete response rates and progression-free survival compared to BEAM alone.
