VD-PACE is an intensive multi-agent chemotherapy regimen primarily utilized for the treatment of aggressive plasma cell dyscrasias, including primary and secondary plasma cell leukemia (PCL) and high-risk or relapsed/refractory multiple myeloma. The regimen incorporates the proteasome inhibitor **bortezomib** (Velcade) and the corticosteroid **dexamethasone** into the established **PACE** backbone, which consists of **cisplatin** (Platinol), **doxorubicin** (Adriamycin), **cyclophosphamide**, and **etoposide**. This combination targets malignant plasma cells through multiple mechanisms, including proteasome inhibition, DNA cross-linking, DNA intercalation, and topoisomerase II inhibition, aiming to overcome the rapid proliferation and drug resistance characteristic of advanced plasma cell malignancies. It is often used as an induction therapy to achieve rapid disease control before proceeding to autologous stem cell transplantation.
