Overview

This combination therapy, frequently investigated in clinical trials sponsored by the University of Athens, consists of the proteasome inhibitor bortezomib (Velcade), the corticosteroid dexamethasone, and the bisphosphonate zoledronic acid (Zometa). It is primarily utilized in the treatment of relapsed or refractory multiple myeloma, with a specific emphasis on managing the malignancy while simultaneously addressing bone-related complications. Bortezomib induces apoptosis in plasma cells by inhibiting the 26S proteasome, while dexamethasone provides synergistic anti-tumor effects and manages systemic inflammation. Zoledronic acid is included to inhibit osteoclast-mediated bone resorption by targeting farnesyl pyrophosphate synthase, thereby increasing bone mineral density and reducing the risk of skeletal-related events (SREs) such as pathological fractures and bone pain.
