Overview

GIFOX + bortezomib (also known as B-GIFOX) is a combination chemotherapy regimen primarily investigated for the treatment of extranodal natural killer/T-cell lymphoma (NKTL), nasal type. The regimen integrates the proteasome inhibitor bortezomib with the GIFOX backbone, which consists of gemcitabine, ifosfamide, and oxaliplatin. The rationale for adding bortezomib stems from the constitutive activation of the NF-κB pathway in NKTL cells, which promotes cell survival and resistance to apoptosis. By inhibiting the 26S proteasome, bortezomib prevents the degradation of IκB, thereby sequestering NF-κB in the cytoplasm and sensitizing cells to the cytotoxic effects of the chemotherapy components. Clinical studies, including Phase II trials, have evaluated its efficacy in newly diagnosed or relapsed/refractory NKTL, showing an overall response rate of approximately 43%, though median progression-free survival has been reported as relatively short.
