Overview

A four-drug combination therapy for multiple myeloma, uniting three established agents—**bortezomib** (a proteasome inhibitor), **lenalidomide** (an immunomodulatory drug), and **dexamethasone** (a corticosteroid)—with **ciltacabtagene autoleucel** (Carvykti), an autologous CAR-T cell therapy targeting BCMA (B-cell maturation antigen) on malignant plasma cells. - **Bortezomib** inhibits the 26S proteasome, disrupting proteostasis in myeloma cells and inducing apoptosis. - **Lenalidomide** modulates the immune system, broadly impacting tumor cell microenvironment and demonstrating anti-angiogenic and apoptotic effects. - **Dexamethasone** offers direct tumoricidal action as a glucocorticoid and suppresses cytokine-induced inflammation. - **Ciltacabtagene autoleucel** is a patient-personalized CAR-T product engineered to recognize and kill BCMA-expressing myeloma cells via T-cell mediated cytotoxicity, cytokine release, and activated immune responses. This combination is studied and used in relapsed/refractory and high-risk newly diagnosed multiple myeloma to exploit complementary mechanisms with the goal of achieving deeper and more durable responses, particularly following multiple prior lines of therapy[2][4][6][7].
